Valley health system program family medicine residency

The residency programs at Chino Valley Medical Center are a part of larger Prime West Consortium sponsoring institution. We are a growing consortium of programs which include:

  • Family Medicine @ Chino Valley Medical Center
  • Podiatry @ Chino Valley Medical Center
  • Dermatology through Chino Valley Medical Center, located in Newport Beach
  • Internal Medicine @ West Anaheim Medical Center
  • Internal Medicine @ Centinela Hospital Medical Center (New 2022!)

We have a few other programs that are in the works and are excited about our growing consortium of programs. We currently have regularly scheduled shared didactic sessions between several of the programs to aid us in delivering high quality educational experiences across the programs.

Prime West Consortium

Prime West is an academic consortium consisting of the residencies within Prime Healthcare Hospitals in California. The goal for each hospital to retain its community characteristics while the consortium provides the support of a larger academic setting. The Prime West Consortium sponsors Family Medicine, Internal Medicine, and Dermatology programs across multiple sites.

Eric Schackow, MD, PhD, FAAFP

Specialties: Family Medicine and Clinical Informatics.

Dr. Schackow completed his Family Medicine Residency at Michigan State University/Genesys Regional Medical Center in Grand Blanc, Michigan. He received his Bachelor of Science and his Doctor of Medicine degrees from Northwestern University Medical School in Chicago, Illinois. He also received his PhD and served as a postdoctoral research fellow in the Department of Molecular Pharmacology and Biological Chemistry at Northwestern University.

Mark Warfel, DO

Dr. Warfel is a graduate of the St. Elizabeth Hospital Family Medicine Program. He received his Doctor of Osteopathy Degree from the University Of New England College Of Osteopathic Medicine in Biddeford, Maine, and a Bachelor of Science in Pharmacy Degree from the Massachusetts College of Pharmacy in Boston, Massachusetts.

Julie Betro-Shkane, DO

Dr. Betro-Shkane received her Doctor of Osteopathy from New York College of Osteopathic Medicine in Old Westbury, NY, completed an Osteopathic Internship at the St. Elizabeth-University of New England College of Osteopathic Medicine (UNECOM) Osteopathic Internship program and her Residency at the Bassett-St. Elizabeth Family Medicine Program and completed her Bachelor of Science in Biology from Utica College.

Affaf Munir, MD

Francis Chabot, MD

Nay Linn Aung, MD

Dr. Aung received his medical degree from the Institution of Medicine-1 in Yangon, Burma. He completed his residency at St. Elizabeth Family Medicine Program in Utica, NY. Dr. Aung also completed the Diabetes Fellowship at East Carolina University in North Carolina.

Hoang Nhu (Natalie) Hua, MD

Dr. Hua practices full spectrum outpatient Family Medicine, including obstetrics and sees patients of all ages at our Sister Rose Vincent Family Medicine Center.  She received her medical degree from American University of Antigua College of Medicine and completed her residency at St. Elizabeth Family Medicine Residency Program in Utica, NY, where she served as Chief Resident.  Dr. Hua earned her Bachelor of Science at Long Island University. 

Goutham Malempati, MD, MHA, FAAFP, SFHM

Dr. Malempati received his bachelors degree and medical degree from Mahadevappa Rampure Medical College (India) and his Masters Degree in Health Care Administration from Utica College, Utica, NY. He is a graduate of St. Elizabeth Family Medicine Residency Program in Utica, NY, a Fellow of the American Academy of Family Physicians and a Senior Fellow of Hospital Medicine.

Andrea Johnson, DO

Afsar Khan, MD

John Leuenberger, DO

He completed his residency at the United Health Services Family Medicine Residency Program and earned his Associates of Science in Biology from Mohawk Valley Community College, Utica, NY and his Bachelor of Science in Engineering and Management at Clarkson University, Potsdam, NY. Additionally, he completed a Neuromuscular Medicine fellowship at Eastern Maine Medical Center, Bangor, ME. He is also certified in Medical Acupuncture

Katherine Warden, PhD

Dr. Warden earned her doctorate in Clinical Forensic Psychology and her Master of Arts in Forensic Psychology at Alliant International University, California School of Forensic Studies in Fresno, California. She earned a Bachelor of Science in Psychology at State University of New York (SUNY) at Cortland in Cortland, New York. Dr. Warden is a licensed psychologist and a Batterer’s Intervention Program (BIP) Facilitator, having completed BIP Facilitator training and domestic violence training. Dr. Warden is the Director of our Behavioral Medicine Program.

Stephen Hudyncia, MD

Sushma Kaul, MD

She completed a fellowship in Critical Care Medicine at the Albert Einstein College of Medicine at Montefiore Medical Center in Bronx, New York, a fellowship in Pulmonary Diseases from St. Luke’s Roosevelt Hospital at Columbia University in New York, New York, a residency and internship in Internal Medicine at Catholic Medical Center, a hospital affiliated with Cornell University in Jamaica, New York, and a rotating internship in Medicine, Surgery and Obstetrics/Gynecology at Shri Maharaja Hari Singh Hospital in Srinagar, India.

Ashok Patel, MD, FACC

Leo Sullivan, MD, FACS

Dr. Sullivan received his medical degree from New York University (New York, NY). He completed an internship and residency in Surgery at New York University (New York, NY) and an additional residency in General/Vascular Surgery at Staten Island Hospital (Staten Island, NY).

Dr. Sullivan is also a Medical Director of our Trauma Center.

Jonathan Wigderson, DO

Dr. Wigderson earned his Doctor of Osteopathic Medicine from the Ohio University College of Osteopathic Medicine in Athens, Ohio. He completed a residency in Orthopedic Surgery at Peninsula Hospital Center in Far Rockaway, New York, and fellowships in Orthopedic Trauma at Mount Sinai Medical Center in New York, New York, and in Orthopedics at Booth Memorial Medical Center in Flushing, New York. He also completed a rotating internship at Mount Clemens General Hospital in Mount Clemens, Michigan.
