Change Your Name

Driver's License or Learner's Permit

If your name has changed due to marriage, divorce or another reason, or if you need to change your address, you will need to complete one of the following forms: 

  • Commercial Driver's License: DL-80CD (PDF), or if expiring in the next 6 months: DL-143CD (PDF) to renew.
  • Learner's Permit: DL-31 (PDF)
  • Non-Commercial Driver's License: DL-80 (PDF), or if expiring in the next 6 months, use DL-143 (PDF) to renew.
  • Photo ID Card: DL-54B (PDF)

A name change for a driver's license or photo ID must be completed in person at a Driver License Center.

If you desire to use your birth name, you must present your state issued birth certificate with a raised seal. If your name changed by permission of court, you must present a Certified Copy of the Court Order. If you desire to use your spouse’s surname, you must present your marriage certificate. If you desire to use another name, you must present your Social Security Card, together with two other sources issued in the desired name such as: Tax Records, Selective Service Card, Voter Registration Card, Passport, any form of Photo I.D. issued by a governmental agency, banking records, or baptismal certificate.

Please Note:  ORIGINAL documents must be presented, such as a court order, marriage certificate, or divorce decree.

Note: Name changes cannot be made for Non-U.S. Citizens unless the primary source ID document(s) reflect that name. The source ID document(s) are considered the most recently issued Department of Homeland Security immigration document(s), not a Passport, Visa, or Marriage Certificate.

Vehicle/Trailer Registration and Title

If your name has changed due to marriage, divorce or another reason, you will need to complete form MV-41A (PDF). Side A of the form should be completed if you are only requesting an updated vehicle/trailer registration card. Side B should be completed if you are requesting an updated vehicle/trailer title.

Change Your Address

Driver's License or Learner's Permit

You can change your address on your non-commercial driver's license or PA photo ID card at any time online, except if you are moving out of state. If you are moving out of state and qualify for a residency exemption for a change of address to your driver's license, please fill out Form DL- 82 (PDF), "Out of State Exemption" and mail it to the address indicated on the form. PennDOT may not issue driver license products to an out-of-state address, except in the case of an employee of federal or state government, armed forces personnel or their families, who workplace is located outside of Pennsylvania. Upon updating your address for your non-commercial driver's license, learner's permit or PA photo ID card,  you will receive a free address update card within 10 days after the change has been made online. This card MUST be carried with your driver's license, identification card, or learner's permit at all times. 

You may also choose to obtain a duplicate non-commercial license or Photo ID for a fee at the time of your address change. Your duplicate product will be issued with the corrected address. Duplicate fees are only applicable when applying for an updated physical product.

Vehicle/Trailer Registration and Title 

You can change your address on your vehicle/trailer registration at any time online or by filling out form MV-63 (PDF), "Change of Address", except if you are moving out of state. To record a change of address on your vehicle registration information, please fill out Form MV-8 (PDF), "Self Certification for Proof of Residency" and mail it to the address indicated on the form.

NOTE: When completing the MV-63, a customer’s voter registration will automatically be updated with the Department of State unless the customer opts out by checking the box in Section D. If a customer is not registered to vote, a voter registration application will automatically be sent to them by the Department of State in the weeks following their submission of Form MV-63.

Address change

By law you have to notify the RMV of a change of address within 30 days of moving. This can be done through phone, online, or in-person.

If you are only changing your address, go online and then write the new residential address using permanent ink on the back of the driver's license or ID card in the area provided, or you can have a driver's license/ID issued for $25.

Name change

If you are applying for a REAL ID driver's license or ID card and your current name doesn’t match the one that appears on your lawful presence document(s), you must prove the legal name change with an acceptable document in order to qualify for a REAL ID credential.   A marriage certificate, divorce decree, or court document all qualify.

If you are applying for a Standard driver's license or ID card, you may change your name with no documentation as long as there is no attempt to defraud.

You must make a reservation at an RMV Service Center to have a new photo and signature captured, and obtain a new driver's license or ID card.

Gender change

If you are changing your gender designation, you will need to fill out an application and make a reservation at an RMV Service Center.

Your gender will appear in the 'Sex' field of your credential as either M, F, or X.

Veteran's indicator

Veterans of the U.S. Armed Forces who were honorably discharged can choose to have the word "Veteran" printed on their MA license or ID card. The veteran's indicator can be added at the time of your license or ID card renewal or when you order a duplicate permit, license, or ID card.

You will need to present a completed license and ID application and one of the following documents to prove eligibility:

  • A DD-214 that indicates honorable discharge; only the long form (full page) is acceptable
  • A DD-215 that indicates honorable discharge
  • An honorable discharge form

Organ donor

When you apply for a Massachusetts driver's license or ID, you will have the opportunity to become an organ and tissue donor. By registering as an organ and tissue donor with the RMV, you will be entered into the Massachusetts Donor Registry, which is legal consent for donation. However, also share your decision to donate with your family and friends so that they know about your decision to become an organ and tissue donor.

Even if you are currently a registered donor, you still need answer "yes" online or on the application you complete in order to remain in the donor registry.

REAL ID or Standard

  1. Decide what type of card you want – a REAL ID driver's license (valid for federal purposes) or Standard Massachusetts driver's license (not valid for federal purposes). Go to mass.gov/id to decide.

  2. If you want a Standard driver's license, you may be able to complete your renewal online by entering the information from your lawful presence document.

  • If you want a REAL ID driver's license, you will need to visit an RMV Service Center with proof of the required documentation  for
    • Lawful presence (1 document)
      • If your name differs on the lawful presence document, you must also provide proof of the legal name change
    • Social Security number (1 document)
    • Massachusetts residency (2 documents)
